The Role

As a Community Care Assistant, you will support clients within their own homes, helping them maintain their independence, dignity and quality of life.

You will provide high-quality person-centred care tailored to each individual’s needs while building trusted relationships with clients and their families.

Your duties will include:

• Supporting clients with personal care, including washing, dressing and grooming
• Assisting with mobility and daily living activities
• Supporting with medication in line with care plans
• Preparing meals and encouraging nutrition and hydration
• Undertaking light household duties, including cleaning and laundry
• Providing companionship and emotional support
• Supporting clients to attend appointments and access their local community
• Encouraging independence and promoting choice
• Maintaining accurate care records
• Reporting changes in clients’ wellbeing to the senior team
• Working closely with colleagues, families and healthcare professionals
